The Outreach services support people in their own home or community to help them to live more independently. Being matched with a Connecting Lives carer or support worker offers consistency and the opportunity to build positive relationships with the same person a few hours a week.

Outreach support is provided by self-employed professional Connecting Lives carers and support workers. They have relevant life experience and training and are committed to help people to live more independently. Connecting Lives carers and support workers come from all family types and all walks of life.

The day support service is for people who live independently or with their family and need extra help and support to achieve their goals. The person is matched with a Connecting Lives carer or support worker. They will aim to build a supportive relationship to help the person to build their confidence and try new activities. This could include:

• learning new skills in the home
• going out in the community
• making new friends and building relationships
• finding new interests
• help to find out what’s happening in the local area

Payments

The outreach day support payment is £14.50 per hour.

The sitting service enables a family carer to have a break by providing a Connecting Lives carer or support worker who can support the person with care needs in their own home. This may include:

• companionship
• support with mobility needs
• support with personal care needs
• support with nutritional needs

Sitting services usually last for a few hours.

Payments

The sitting service payment is £14.50 per hour.

Connecting Lives carers and support workers provide care and support in holiday accommodation to provide short breaks for people with care and support needs.
