SEND Hub - education, health and care plans

An education, health and care plan (EHCP) is a legal document that describes a child or young person’s special education, health and social care needs.

An EHCP:

  • is for children and young people with SEND from birth to 25 years if they are in education or training
  • contains information about what support the child or young person needs across education, health and social care
  • explains how that support will be given to them and the goals they could achieve with this support
  • is made with the child or young person and their family so they can have a say in what goals they want to achieve
