SEND Hub - Register of Additional Needs and Disabilities

You can use this page to find out about the Register of Additional Needs and Disabilities and how to register your child.

The Register of Additional Needs and Disabilities, also known as the Disabled Children’s Register, is not just for children who have a disability. The register is for all young people aged from birth to 25 who have special educational needs and disabilities (SEND).

Registering your child is voluntary, but it helps us to think ahead about the capacity we need to provide in schools and other services.

Register your child

Before you start:

  • you need to be a resident of North Somerset to register your child or young person
  • being on the register does not mean you will automatically get support - find out what support is available for parents and carers
  • you will need your child’s NHS number
  • if you want to review or change your information, please log in to manage or delete your record

Your data

The information you give us will be used to plan services and resources for children with special educational needs and disabilities (SEND).

You are protected by the Data Protection Act 2018, which means:

  • no specific information on individuals will be shared with other organisations
  • statistics we share with our partners in the NHS and education sector will not have identifying information like names
  • we will ask for your child’s NHS number because this is a unique number which helps us know whether we have any duplicated records - we will not use this number to access any of your child’s medical records
  • the Disabled Children's Register is not linked in any way to NHS patient record systems
  • sharing information with other professionals or organisations will only happen if we are legally obliged and follows the strict rules of the Data Protection Act 2018
  • by completing this form, you are giving your consent for North Somerset Council to process your information so we can register your details on the Disabled Children's Register
