Application forms

The following forms can be used to make a licensing application to North Somerset Council.

All our forms are in Word format and where possible they should be completed online before being returned to our Licensing Team.

For applications involving premises licences please use the following relevant forms:

• Application for a premises licence
• Application to transfer premises licence
• Application to vary a premises licence
• Application to vary a premises licence to specify an individual as designated premises supervisor
• Consent of an individual being specified as a premises supervisor
• Consent of premises licence holder to transfer
• Interim authority notice
• Application for a Provisional Statement

For applications involving personal licences please use the following relevant forms:

• Application for a personal licence
• Disclosure of convictions and declaration
• Photograph endorsement

To make a Temporary Event Notication please use the following form:

• Temporary Event Notice (TEN)

For applications involving club premises certificates please use one of the following relevant forms:

• Application for a club premises certificate
• Declaration for a club premises certificate
• Application to vary a club premises certificate

To make an objections or request a review of a licence please use the following form:

• Application for a review of a premises licence or club premises certificate
