The following forms can be used to make a licensing application to North Somerset Council.

All our forms should be completed and returned to our Licensing team.

For applications involving premises licences please use the following relevant forms:

• Application to vary a premises licence
• Application to vary a premises licence to specify an individual as designated premises supervisor
• Consent of an individual being specified as a premises supervisor
• Consent of premises licence holder to transfer
• Interim Authority Notice
• Application for a Provisional Statement
• Application for a minor variation to a premises licence
• Application to remove the requirement to have a premises supervisor
• Application from an individual to be removed as the premises supervisor

To make a Temporary Event Notication please use the following form:

• Temporary Event Notification (TEN)
• Application for a club premises certificate
• Declaration for a club premises certificate
• Application to vary a club premises certificate

To make an objections or request a review of a licence please use the following form:

• Application for a review of a premises licence or club premises certificate
