need to do is complete the relevant form and send it to
us.

page, or pick one up from your
local
library or the Town
Hall main reception in Weston-super-Mare.
Over 60 application:
Form only,
information booklet (including form),

Disabled application:

You can return your completed forms to:
- any of our libraries
- housing offices
- Council Connect at the For All Healthy Living Centre
- Town Hall
Please note that upon receipt of your application your Diamond
Travelcard will be processed and posted out to you within two
weeks.
