Partners and project spurs

The North Somerset POPP is made up of five spurs which, with the help of our partners, will develop local services for older people.

The Department of Health (DoH) awarded North Somerset Council and partners almost £1m to develop positive and innovative services for older people across the district.

For the purposes of POPP an older person is defined as someone aged 50 years or older.

The North Somerset POPP is made up of the following five spurs which are run in conjunction with specific partners:

• Age Concern Somerset Project - Introduction of four community development workers and Team Leader to help older people engage with services
• Integration of Health and Social Care Services Project - Development of working practices for health, mental health and social care services to support staff to work in an integrated way to benefit older people
• North Somerset Crossroads Caring for Carers Project - Introduction of flexible services to help older carers access a wider range of services and alleviate strain
• Pavement Repair Project - The repair of key areas of pavement to reduce injuries and associated costs and distress
• Somerset Racial Equality Council (SREC) Project - Introduction of a community development worker to engage older members of the local Black and Minority Ethnic (BME) community

We will also link with Weston Area Health NHS Trust, local GPs and other interested parties throughout these projects.
