This project aimed to change the way community
services work for the benefit of older people across
North Somerset.
POPP started the development of a single point of access
for community health and social care services including mental
health services for older people.
This single contact point is continuing to develop with
Care Connect
and people needing advice and support on a wide range of health and
social care issues can now contact this one single number.
Health,Social Care and Mental Health services are provided
separately by
Avon and Wiltshire Mental Health
Partnership NHS Trust, North
Somerset Primary Care Trust (PCT) and North Somerset
Council Adult Social Services and Housing (ASS&H)
department.
This project has helped staff from these organisations work
more closely, to reduce the duplication of services and provide the
most appropriate support when it's needed.
