We aim to have implemented the lead professional
role across North Somerset by the end of 2008.
We are working towards this by identifying lead professionals
for children who have had a
Common Assessment Framework (CAF) and have a plan involving
more than one agency.
We are also running a lead professionals pilot for children with
complex disability involving professionals from our Children and
Young People's Services,
North Somerset Primary Care
Trust, Springboard Opportunity
Group and Weston Area Health
Trust.
