Exemplar questionnaires - Secondary schools
Broadoak Community School sampled the views of Year 7 students
and their parents and carers when planning provision.

Primary schools
Two questionnaire templates have been developed for primary
schools.
For schools which have an after-school or breakfast club in
place and want to point it out to parents please download our
Extended Services Parents Questionnaire.
For schools which do not have them in place and would like to
find out about possible demand from parents please download our
Additional Services Parents Questionnaire.
This questionnaire asks parents whether they would want to
access breakfast or after-school clubs at neighbouring schools.
We recommend our questionnaires are used in conjunction with an
event which involves parents rather than being sent out to parents
as this is likely to secure a much higher response rate.
We provide support for schools in surveying parents and
collating the results with further information available from our
Senior Childcare Development Worker on 01275 888 610.

Hiring and letting agreements
We provide guidance on hiring and letting procedures through our
Extended
Schools Toolkit.
You will mainly have to decide whether to make an agreement to
let a room to a third party or may be advised to enter into a
Transfer of Control Agreement.
