We are working with local partners to develop a
Disabled Children's Service to deliver services for children with
complex disabilities.
Alongside
North Somerset Primary Care
Trust and Weston Area Health
Trust, we aim to set up an integrated service delivering
seamless specialist support for disabled children.
It will establish care pathways for specialist workers and
universal services, such as schools and health visitors, allowing
them to work together to meet children's needs.
It will provide a single point of access for services allowing
them to share best practice, budgets, information and locations
where appropriate.
It will help implement the new
lead
professional role and deliver services in line with key
national standards and guidance.
It will also help achieve a number of the goals identified in
the Disabled Children's Strategy.
