Intermediate care FAQs

What do you offer?

We offer rehabilitation for every day activities such as getting dressed, as well as practical help with tasks until you are able to do them independently. We also give advice on how to adapt routine tasks to enable you to manage independently. We act as a liaison with other community services (including district nurses, GPs, Social Services) to provide further ongoing help where appropriate.

Who is eligible for intermediate care?

We offer our service to adults aged 18 and over who are resident in North Somerset. Are aged 18+. Have an illness or a disability which has caused a loss of independence and/or deterioration of health and are unable to carry out day to day personal and social care activities. Would benefit from a short term (up to six weeks) intensive intervention of multi-disciplinary rehabilitation services. Agree to the service provision and have been referred above.

What are your aims?

We aim to speed up hospital discharges and to prevent unnecessary admissions to hospital or care homes. North Somerset Intermediate Care Service is a partnership service between health and social care services. The social care is provided by North Somerset Council (Adult Social Services and Housing) and the health care is provided by North Somerset Primary Care Trust. The aim of the Intermediate Care is to provide a service that maximises your potential for health and independence. We aim to provide a service to meet your needs. This can be provided to adults resident in North Somerset who are experiencing a reduction in their daily living skills; often as a result of ill health or injury

Do you charge for your service?

The therapy service provided by intermediate care staff is free but is time limited and dependent on assessed need. If you need home care, this will be subject to our Home care charging policy. A financial assessment will be offered.
