We are a community based service who give assistance to people
with rehabilitation needs. This includes giving advice on how to
manage at home.
Our team includes social workers, occupational therapists,
physiotherapists, home care staff and intermediate care assistants
with access to nurses.
The aim is to give short-term assistance so you can manage
independently from as little as a few days up to a maximum of six
weeks.
The service is free but time limited and open to adults aged 18
and over who are experiencing reduced ability and independence as a
result of physical illness or injury and who would benefit from a
short period of rehabilitation.
The therapy service provided by intermediate care staff is free
but is time limited and dependent on assessed needs.
If you need home care, this will be subject to our
Non Residential Services charging
policy. A financial assessment will be offered.
The help you receive will depend on your needs as assessed and
reviewed by our team. These services can include the following:
- rehabilitation for every day activities such as getting
dressed
practical help with tasks until you are able to do them
yourself
- dvice on how to adapt routine tasks so you can manage
independently
- liaison with other community services like district nurses, GPs
and social services
These services are available within your home, following
discharge from hospital and in order to prevent an unnecessary
admission to hospital or residential care.
All staff are employed by
North Somerset
Primary Care Trust or North Somerset Council.
