  • How can I find out about the local hospice and what kind of services they offer?

    Care Connect can send you a leaflet with more information on the local hospice. Alternatively, you can call Weston Hospice Care on 01934 423 900 or visit its website.

  • If there is a limited prognosis when should I call social services and when should I call the primary care trust for home care?

    When the prognosis is less than eight weeks, the continuing health care team at the primary care trust will be responsible for care packages and equipment needs - you can call them on 01275 546714. More than eight weeks is handled our Care Connect service on 01275 888 801.

  • What benefits might I be entitled to?

    You may be entitled to the disabled living allowance or attendance allowance depending on your needs. Contact our Care Connect service on 01275 888 801 who will refer you to the correct department or arrange for someone from the Depart of Work and Pensions to visit you for a benefits check.
