A care assessment will help us decide what your
needs are and how they can be met.
They are carried out through discussions between
yourself and one of our care managers, social workers or
occupational therapists.
These talks cover your personal circumstances, the extent of your
disability, the help you have available and whether other people
can help.
The answers to these questions will help us decide whether we can
offer you our care and most assessments are very
straightforward.
If you have complex problems a more detailed assessment will be
required and with agreement we can include your doctor or community
nurse.
However, there will be some situations when we will not be able to
give you all the help you may feel you need.
If you do receive our help this will be described in a care plan,
which you will receive a copy of, and any costs will be explained
to you.
One of our officers will carry out a financial assessment and
discuss ways you can pay and give advice on benefits.
