are self inflicted) and mental health issues?
Do the service users lead a sedentary lifestyle?
You could refer the service user to the Go4Life Chequebook
Scheme; the aim of this intervention is to increase
physical activity levels of service users by using local
leisure centre
facilities to get active.
If you support service users experiencing health inequalities
you can become a Chequebook Scheme key worker by reading and
completing the
information pack and key worker agreement
form.
To refer service users experiencing health inequalities to the
Chequebook Scheme, the
non user application form should be fully
completed and returned once a taster voucher has been used.
