Making a referral

Any school making a referral must do so in conjunction with appropriate Children's Services Authority officers such as Assistant Locality Leads (learning) Education Welfare Officers, Educational Psychologists, Inclusion Advisory Team, SEN Officers.

For a referral to be considered for placement, the following information should be enclosed with the referral form;

  • Copies of the students last two years attendance data
  • Copies of at least two individual student pastoral support programmes and reviews
  • Individual student provision map
  • Student conduct log
  • Evidence to show the student is School Action Plus (SAP)
  • Copies of reports from other agencies working with the student

The needs of statemented students should be discussed at an annual review or with the SEN Officer before the referral is made.

Placement within Education Other Than At School provision (EOTAS) will be agreed at the EOTAS panel (which meets twice per term) and is dependent on the needs of the child/young person.
